- Clinical judgement should always override the score — e.g. hypoxemia, social factors, or inability to tolerate oral intake may still warrant admission at a low score.
- Risk class I (the lowest class) is only assigned when age < 50 with no listed comorbidities and normal vitals; otherwise the point total determines class II-V.
