SOFA Score

Quantifies organ dysfunction across six organ systems.

What is SOFA Score?

Quantifies organ dysfunction across six organ systems. SOFA Score is intended for use in: Adults.

How to Use the SOFA Score Calculator

  1. Enter PaO2/FiO2 ratio in mmHg.
  2. Enter On mechanical ventilation / respiratory support.
  3. Enter Platelets in ×10³/µL.
  4. Enter Bilirubin in mg/dL.
  5. Enter Mean arterial pressure (if no vasopressors) in mmHg.
  6. Enter Vasopressor support.
  7. Enter GCS.
  8. Enter Creatinine in mg/dL.
  9. Enter Urine output (if oliguric) in mL/day.
  10. Click Calculate to see your result and interpretation.

Formula & Version

This calculator uses formula version 1.0, last reviewed September 2026.

Clinical Considerations & Limitations

  • The cardiovascular component's vasopressor-dose bands are simplified into categorical bands here for usability rather than exact mcg/kg/min thresholds.
  • Renal component uses whichever of creatinine or oliguria gives the higher (worse) point score, per the original scoring convention.
  • Scoring tables independently re-verified against Vincent 1996 during the STEP 4 QA review; a respiratory sub-score boundary bug (ventilation-dependent escalation applied one bracket too early) was found and fixed.
